Gently apply a 2x2 gauze to lightly dry out the mucosa. You don't want to dessicate it; just remove the excess saliva
Apply the topical gel. We're using Ultradent's benzocaine gel in Walterberry flavor. Which is not actually a berry.
Cover the swab of topical and the gauze and have the patient close their mouth to hold both in place for at least one full minute, ideally 2.
Here is the 8.4% bicarbonate solution we use, along with the 0.3cc insulin synringe from BD and the lidocaine carpule.
Wipe the bicarbonate with an alcohol soaked gauze to clean it.
Wipe the lidocaine carpule with an alcohol soaked gauze to clean it.
Use a 0.3cc diabetic insulin syringe.
Inject 7 iu of air into the bottle.
Withdraw 7 iu of bicarbonate solution for lidocaine.
Insert the insulin syringe into the lidocaine carpule
After you inject the sodium bicarbonate, the stopper will slightly protude from the lidocaine.
Remove the gauze from the patient's mouth and Inject as usual. Between the extra long topical and the buffered lidocaine, the injection will be less painful, often painless.
